Write down the things that you do to help keep you from relapsing to drugs or alcohol. They must be behaviors not ideas, for example: Attend 12 Step meetings vs. be strong.
Review this chart at the beginning of each week and check off the behaviors you have maintained from the previous week to stay clean and sober. Do the same for the bottom portion only these are behaviors to avoid.
If you find that you are missing 2 or more checks from each section it indicates that you are drifting towards use.
